* Telephone requests are NOT accepted
* Transcripts can be mailed or picked up
* Use same forms & process below to request Immunization Records
* Former Student's Signature is required
* Process time is approximately 3 to 5 days (NO expedited services available)
* We cannot accept requests from a second party without signed consent from the former student
(this includes parents of students 18 years and older )
